Institute for Exceptional Care (IEC) is excited to share that we have started testing for the Always Uniquely Me app in partnership with Northwell Health on Long Island, NY.

We are looking for up to 1,200 people with intellectual and/or developmental disabilities (IDD) on Long Island to join us in this important project.

What is Always Uniquely Me?

Always Uniquely Me is a mobile app designed with people with intellectual and/or developmental disabilities (IDD) and clinicians, to support better and safer medical visits – especially in high-stress environments like emergency rooms.

Always Uniquely Me includes things like:

  • Snapshot of Me – A short video that shows who the person is when they are at their best.
  • Tips for Taking Care of Me – Calming strategies, sensory activities, and triggers to avoid.
  • My Medical Information – Key medical history and emergency details tailored to ER teams.
  • And much more!

This way, clinical teams get to know the person behind the patient and can give better care that respects everyone’s unique needs.

Why participate?

  • Share your voice: Help doctors and nurses understand people with IDD better
  • Improve care: Your feedback will make hospital visits easier and safer for others
  • Be part of change: Together, we can show how health care can work better for people with IDD

Who can join?

We are inviting adults with IDD on Long Island. Family members and other caregivers can also help.
